Making Appointments

Making an appointment may be made online or by calling us directly. For R5 Skinworks there is a 36 hour cancellation policy, otherwise 50% will be charged.

Arrival

Please arrive 15-20 minutes prior to your scheduled appointment time. Starting your service on time allows you the full relaxing experience you deserve. Your welcome forms, spa robe, and a warm aromatherapy neck pillow will be waiting for you. Make the most of your spa experience by relaxing in our retreat room before and after your services. All treatments will end on time. After your R5 Skinworks experience, please drink plenty of water.

Cell phones & Personal property

To maintain an atmosphere of tranquility, please turn off your cell phone or other communication devices. Cell phones and Pagers are only allowed in the dressing area of the treatment rooms. R5 Skinworks and employees, or independant consultants, are not responsible for lost or damaged personal property.

Payment Options

We accept cash, checks, Visa, Amex and MasterCard. There will be a $20 charge for all returned checks. Prices and treatments are subject to change without notice. Gift cards may not be used with other offers, promotions or discounts.

Gift cards, Product and Membership purchases

Gift cards are available in any denomination and for any treatment. Gift cards are non-refundable. We also offer a variety of products to purchase for at- home use. Sanitas may be returned within 30 days with a receipt or spa credit without receipt. Make-up and other product offerings are final sales.

Promotional gift cards are accepted but may not be used in conjunction with any other offers, promotions or discounts. Memberships are for a minimum of three months and may not be transferred. Memberships are non- refundable and non-transferrable. Members may not use any discounted bring-a-friend or discounted gift card for personal use at any time; before, during or after their membership. Memberships must be canceled in writing, with 30 days notice, on a R5 Skinworks cancellation form. If written cancellation is less than thirty days of auto-renewal, the cancellation will be in effect the following month.

Cancellations

In consideration of other guests and our specialists, we kindly ask that you notify us of any cancellation 24 hours in advance of a single service and 24 hours in advance of multiple services. R5 Skinworks cancellations must be made 36 hours in advance, otherwise 50% will be charged. All services must be reserved with a credit card. We reserve the right to bill half of the amount for missed appointments or for appointments canceled less than 24 hours in advance. Our policy is designed to respect other guests and to provide compensation to our employees, who rely on us to manage their schedules and their careers with R5 Skinworks. Memberships must be canceled in writing 30 days before the renewal date.

Gratuity

Gratuity is not included in the cost of your spa treatment, but is greatly appreciated by your aesthetician. If you enjoyed your service, it is customary to leave gratuity based on the value of the treatment received. Please leave gratuity in the form of a personal check or cash. Gratuity envelopes can be provided at checkout within your treatment room. For parties of 5 or more, 18% gratuity will be added to each service.

Special Accommodations

If you have any ailments, allergies, sensitivities, disabilities or special health concerns, including pregnancy, please let us know in advance so we can best accommodate you. For the safety of mother-to-be and baby, we are only able to provide treatments in the second and third trimesters with your Doctor’s approval.

Children

Parents and guardians must sign a consent form for any spa guest under the age of 18. We do not offer lash extensions to children under 16 years of age.

We ask for assistance in ensuring that they respect the quiet, relaxing environment of the spa.

Return Policy

Any products purchased have a 30 day guarantee and either a reimbursement or a credit will be applied to your account, whichever you prefer. Services and Products purchased online must be paid in full. Services require a reschedule or early cancellation prior to 24 hour of your appointment and a full refund if cancelling, or credit will be applied to your account if you are rescheduling.

WAIVER AND RELEASE OF LIABILITY

By having an appointment scheduled with Martha Smithers-Ross at R5 Skinworks, or scheduling to participate in any activity/event outside the storefront location, that I, as a client, agree to the following:

In consideration of the risk of injury while participating in(the “Activity”), and as consideration for the right to participate in the Activity, I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives, knowingly and voluntarily enter into this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation in the Activity, and do hereby release and forever discharge , located at 410 S Comet Ave, Martha Smithers-Ross, R5 Skinworks, LLC, any affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, for any physical or psychological injury, including but not limited to illness, paralysis, death, damages, economical or emotional loss, that I may suffer as a direct result of my participation in the aforementioned Activity, including traveling to and from an event related to this Activity.

I AM VOLUNTARILY PARTICIPATING IN THE AFOREMENTIONED ACTIVITY AND I AM PARTICIPATING IN THE ACTIVITY ENTIRELY AT MY OWN RISK. I AM AWARE OF THE RISKS ASSOCIATED WITH TRAVELING TO AND FROM AS WELL AS PARTICIPATING IN THIS ACTIVITY, WHICH MAY INCLUDE, BUT ARE NOT LIMITED TO, PHYSICAL OR PSYCHOLOGICAL INJURY, PAIN, SUFFERING, ILLNESS, DISFIGUREMENT, TEMPORARY OR PERMANENT DISABILITY (INCLUDING PARALYSIS), ECONOMIC OR EMOTIONAL LOSS, AND DEATH. I UNDERSTAND THAT THESE INJURIES OR OUTCOMES MAY ARISE FROM MY OWN OR OTHERS’ NEGLIGENCE, CONDITIONS RELATED TO TRAVEL, OR THE CONDITION OF THE ACTIVITY LOCATION(S).

NONETHELESS, I ASSUME ALL RELATED RISKS, BOTH KNOWN OR UNKNOWN TO ME, OF MY PARTICIPATION IN THIS ACTIVITY, INCLUDING TRAVEL TO, FROM AND DURING THIS ACTIVITY.

I agree to indemnify and hold harmless against any and all claims, suits or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my behalf, including attorney’s fees and any related costs, if litigation arises pursuant to any claims made by me or by anyone else acting on my behalf. If incurs any of these types of expenses, I agree to reimburse. I acknowledge that and their directors, officers, volunteers, representatives and agents are not responsible for errors, omissions, acts or failures to act of any party or entity conducting a specific event or activity on behalf of R5 Skinworks, LLC

I ACKNOWLEDGE THAT THIS ACTIVITY MAY INVOLVE A TEST OF A PERSON’S PHYSICAL AND MENTAL LIMITS AND MAY CARRY WITH IT THE POTENTIAL FOR DEATH, SERIOUS INJURY, AND PROPERTY LOSS. The risks

may include, but are not limited to, those caused by terrain, facilities, temperature, weather, lack of hydration, condition of participants, equipment, vehicular traffic and actions of others, including but not limited to, participants, volunteers, spectators, coaches, event officials and event monitors, and/or producers of the event.

I ACKNOWLEDGE THAT I HAVE CAREFULLY READ THIS “WAIVER AND RELEASE” AND FULLY UNDERSTAND THAT IT IS A RELEASE OF LIABILITY. I EXPRESSLY AGREE TO RELEASE AND DISCHARGE AND ALL OF ITS AFFILIATES, MANAGERS, MEMBERS, AGENTS, ATTORNEYS, STAFF, VOLUNTEERS, HEIRS, REPRESENTA-TIVES, PREDECESSORS, SUCCESSORS AND ASSIGNS, FROM ANY AND ALL CLAIMS OR CAUSES OF ACTION AND I AGREE TO VOLUNTARILY GIVE UP OR WAIVE ANY RIGHT THAT I OTHERWISE HAVE TO BRING A LEGAL ACTION AGAINST FOR PERSONAL INJURY OR PROPERTY DAMAGE.

To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence on the part of R5 Skinworks, Martha Smithers-Ross, its agents, and employees. In the event that I should require medical care or treatment, I agree to be financially responsible for any costs incurred as a result of such treatment. I am aware and understand that I should carry my own health insurance. In the event that any damage to equipment or facilities occurs as a result of my or my family’s willful actions, neglect or recklessness, I acknowledge and agree to be held liable for any and all costs associated with any actions of neglect or recklessness.
